Convening Registration

REGISTRATION

NOTE: Convening registration fees include all materials as well as continental breakfast and lunch on May 20-21.


All payments must be received by April 26, 2010.  Payments will not be accepted at the Convening.


 


Registration Fees


NOTE:  Registrations received with payment after April 5th are subject to the higher standard registration fee.


CLINIC Affiliate* (Early Bird rate until April 5):  


                   $275 per person  


                   $825 per office cap for 3 staff from the same office address


                   $225 per person for additional staff (after first 3 staff members from the same agency have registered under the Early Bird rate)


CLINIC Affiliate (after April 5):


                   $350 per person


                  $1,050 per office cap for 3 staff from the same office address


                  $275 per person for additional staff (after first 3 staff have already registered)


*A CLINIC Affiliate is defined as a dues-paying member or subscriber.


Non-profit organizations & organizations under MOU with CLINIC*:


                  $395 per person


                  $1,185 per office cap for 3 staff from the same office address


                  $360 per person for additional staff (after first 3 staff have already registered)


*Organizations with 501(c)(3) status, please also send or faxt a copy of your IRS designation letter. Your registration will not be processed without this letter.

Please note that registrations received without payment will not be processed and registrations will not be confirmed until full payment has been received. 


On receipt of full payment, a registration confirmation will be sent via e-mail.


Community-based organizations with 501(c)(3) status: Please fax or mail a copy of your agency's federal IRS 501(c)(3)  designation letter and a copy of your registration form to CLINIC. We CANNOT process your registration without the designation letter.

Refunds/Cancellations


Registration fees  will be refunded, less a $100 processing fee, with written notification no later than Monday, April 9th.  Refunds WILL NOT be given for no-shows.
